* Provides short-term crisis intervention services to help reduce emergency admissions through early crisis intervention, as an alternative to calling the police or driving to a psychiatric emergency room when an individual is in crisis. Services provided by the Team will include:
- Crisis triage and assessment, stabilization and referral linkage.
- Travel to homes, schools, work-sites and other locations in Nassau County.

Eligibility: Resident of Nassau County.
Application Process: Call and a social worker will take some information and a member of the team will call back for a brief telephone intake.
Required Documentation: Call for information.
Fees: No fee for service.
