* Provides mental health engagement, intervention and follow-up support to help people remain connected with treatment providers. The teams may offer a range of services, including: 
  • Assessment. 
  • Crisis intervention. 
  • Supportive counseling. 
  • Information and referrals, including to community-based mental health services.
