* Provides comprehensive, community-based program for individuals diagnosed with serious mental illness whose needs have not been met by traditional, office-based services.

* Provides services by team of clinicians, including:

  • Mental health evaluation and treatment.
  • Substance abuse evaluation and treatment.
  • Employment services.
  • Case management.
  • Family psycho-education and support.
  • Life-skills acquisition.
  • Housing.
  • Transition planning.

Eligibility: Resident of area served. Ages 18 and older with a diagnosis of major mental illness.

Application Process: All referrals are done through SPOA, https://www2.monroecounty.gov/mh-adult-spoa-referrals.

Fees: Medicaid.

Coverage Note: Monroe County.
