* Provides care management and support services including:

  • Meet your personal health goals and outcomes.
  • Coordinate transportation to medical visits.
  • Understand your medical needs, even if they are complex, and connect you to the care you need.
  • Provide insurance assistance (ACA Marketplace, ADAP, private insurance, Medicaid and Medicaid spend-down, Medicare Part D).
  • Attend medical appointments and pharmacy visits for medication follow-up.
  • Find primary care and other kinds of medical care.
  • Coordinate referrals and follow through for mental health and substance abuse.
  • Explore other services, like nutrition education, day health, our food pantry, and more.
  • Housing assistance and planning.
  • LGBTQ-focused Care Management services

Fees: Case management: Medicaid billed or no charge. Other services: no fee for service.

Coverage Note: Monroe, Livingston, Ontario, Schuyler, Seneca, Steuben, Wayne and Yates Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff, Staff reflects population served

Languages: American Sign Language, Spanish - Staff provides in-person interpreter, Language line
