Primary care services for all children in Monroe County family based foster care:

    • Well Child Check (annual physical)
    • New to care (new patient to practice)
    • Return to care (returning to practice)
    • Follow up appointment
    • Medication management appointment
    • Acute-Urgent/acute need (sick visit)
    • Immunizations only visit

Eligibility: Monroe County DHS foster care children birth to 21 years of age.

Application Process: Referral from Monroe County Department of Human Services foster care system only.

Required Documentation: Call for information.

Fees: Accepts all insurance including Medicaid, Medicaid Managed Care.
