* Offers Fiscal Intermediary Services to individuals approved by the Office for People With Developmental Disabilities for Self-Direction or Community Transition Services.
- Fiscal Intermediary Services complete the billing and payment of NY State and Medicaid funded services identified in the approved budget and acts as the Employer of Record for plans with Self-hired Staff, including HR and Payroll functions.
Eligibility: Individuals with a developmentally disability who are approved for services through the OPWDD and are in enrolled in HCBS Waiver services.
Application Process: Complete intake application located on website.

Fees: Must be Medicaid-eligible.
Coverage Note: Monroe County.
Trinity Assistance Corporation provides supports for people with developmental and intellectual disabilities. The supports provided are all non-certified, community based supports. Services included are Respite, Community Habilitation and Fiscal Intermediary services.
