* Offers a 18 bed co-ed State Operated Community Residence. Individuals residing in the community must continue mental health care. Residents work to build skills that will enable them to live in a more independent setting.

Eligibility: Serious persistent mental illness. 18 years of age or older.

Required Documentation: SPOA (Single Point Of Access) referral.
Fees: Sliding scale, Medicaid, Medicare. Fee may be waived according to need.
Coverage Note: Monroe, Genesee, Livingston, Ontario, Orleans, Wayne and Wyoming Counties.
