* Provides community mental health clinic services for adults and children. Services provided include: 
  • Individual mental health therapy for children (starting at age 4), adolescents, and adults. 
  • Group therapy. 
  • Medication management. 
  • Nursing and care management services.

Required Documentation: Photo ID, Proof of Address, Income, and Insurance.
