Eligibility: Age 18 or older. Must meet the SMI criteria for each program and additional documentation is required to go with each referral. It is recommended that family, individuals, and or providers contact SPOA directly to discuss a possible referral. Call for information.

Required Documentation: A referral is required for each service that can be accessed on the Monroe County website https://www.monroecounty.gov/mhadult-spoa.

Fees: No fee for services.
