- For persons who use wheelchairs for medical, social, recreational or personal reasons.
- Nutritional sites.
- Foster care children.
- Cancer patients.
- Developmentally disabled adults to day treatment programs.
- Patients undergoing radiation treatment at area oncology departments.
- Dialysis services.
- Area social service agencies for program-related activities.

Eligibility: Disabled and have no other means of transportation. Must be able to travel with other patients.
Application Process: Self referral. Medical referral through Social Service agencies, hospital, clinic or physician. Call in advance for program-related activities. Medicaid recipients: Medicaid Prior Authorization through Medical Answering Service. Physician completed and submitted authorization form. Call for information.

Fees: Rates vary. Call for information. Reduces fares available if qualified based on poverty line income. Private pay. Third party insurance in cases referred by insurance company. Medicaid. Medicare does not pay for these services.
