  • Diet, exercise, emotional well-being initiatives. 
  • Connection to local wellness resources. 
  • Crisis intervention in times of need. 
* Assesses personalized case management through the Edward M. Dowd Personal Advocate Program.

Eligibility: Must have MS or caring for someone who has MS.

Required Documentation: Documentation of diagnosis may be requested.

Fees: No fee for services.

Coverage Note: 50 Counties in Upstate NY.
