* Provides in-home support services for adults with complex needs to reside in their own independent apartments.
  • Go into home to provide services.
  • Go with client into community to provide life-skills.
  • Liaison with other health providers including:
    • Peer bridgers.
    • Community living coaches.
    • Financial case manager.
  • Linkage to community education.

Eligibility: Resident of supported housing reinvestment beds (RIV). Age 18 or older.

Application Process: RIV: Referral through SPOA. Non RIV: call for information.

Required Documentation: Completed application.

Fees: No fee for service.

Coverage Note: Monroe, Livingston, Wayne, Wyoming, Genesee and Orleans County.
