Medicaid-funded program that allows people with disabilities to hire and train personal assistants to care for them in their homes. CDR serves as a fiscal intermediary, processing payroll for personal assistants and providing support to consumers as they manage their own program of care. 

Eligibility: Must be Medicaid eligible, currently receiving or approved for personal assistance, expected to need services for at least 180 days, be self-directing or have someone who can be responsible for the program on your behalf.

Coverage Note: Allegany, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Tioga, Wayne, Wyoming and Yates Counties. Some restrictions apply in some counties.
