Provides information and referral, independent living skills training, peer support services, advocacy, youth transition and community integration services, and housing, to support and promote community living for people with disabilities in the setting they chose.  

Eligibility: Individuals with disabilities. Self-disclosure of disability is required.

Fees: No fee for services.

Coverage Note: Monroe, Livingston, Ontario, Wayne, Yates and Seneca Counties.
