Eligibility: Income eligible parent or legal guardian are those meeting WIC, DSS, SSI, Medicaid, SNAP, or Head Start income guidelines as outlined at https://www.health.ny.gov/prevention/nutrition/wic/eligibility.htm.

Application Process: Our referral form can be filled out by a case manager or social service agency. You can also fill the form yourself with eligibility verification questions. Once referral is sent into the Office of Traffic Safety, we will reach out for scheduling.

Required Documentation: Proof of address, proof of income.

Fees: No fee for services.
