* Offers Community Health Services for individuals who are currently incarcerated. 
  • Outreach and education to prevent the spread of HIV, Hepatitis C, and STI's, including peer training. 
  • Free testing for HIV, STD's, and Hepatitis C. 
  • Assists individuals living with HIV & Hepatitis C in developing a care plan for when they transition back to community.

Eligibility: Must be currently incarcerated.

Application Process: Call for information. DOCCS referrals to program.

Required Documentation: DOCCS referral information.

Fees: No fee for service.

Coverage Note: Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence Counties.

Languages: English - Staff provides service in language offered, Language line

 

ACR Health is a not-for-profit, community-based organization providing case management, HIV/HCV/STI testing, prevention education, linkage to care, harm reduction services, insurance navigators, and a variety of support services to a wide range of chronic diseases, including focus areas related to insurance, testing, substance use, and HIV, with the goal of positive health outcomes for individuals and communities.
