* Offers transportation assistance for individuals living with HIV when they cannot be met by Medicaid Transportation.

* Ensures access to medical appointments, social security, and HIV related services. Individuals are provided cab rides, bus tokens or gas cards to meet with their medical providers.

Eligibility: Individuals who are living with HIV and have unmet transportation needs to access care.

Application Process: Can do walk-ins, but prefer to operate by appointment. Call for information.

Required Documentation: Call for information on proof of eligibility.

Fees: No fee for services.

Coverage Note: Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff

Languages: English - Staff provides service in language offered, Language line

 

ACR Health is a not-for-profit, community-based organization providing case management, HIV/HCV/STI testing, prevention education, linkage to care, harm reduction services, insurance navigators, and a variety of support services to a wide range of chronic diseases, including focus areas related to insurance, testing, substance use, and HIV, with the goal of positive health outcomes for individuals and communities.
