* Offers nutritional education sessions for individuals living with HIV, as well as group education and meal planning assistance.

* Offers food assistance through pantry bags and food vouchers for healthy foods.

Eligibility: Individuals who are living with HIV and have unmet nutritional needs and require nutritional counseling services.

Application Process: Can do walk-ins, but prefer to operate by appointment. Call for information.

Required Documentation: Call for information on documentation of eligibility.

Fees: No fee for services.

Coverage Note: Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence Counties.

Languages: English - Staff provides service in language offered, Language line

 

ACR Health is a not-for-profit, community-based organization providing case management, HIV/HCV/STI testing, prevention education, linkage to care, harm reduction services, insurance navigators, and a variety of support services to a wide range of chronic diseases, including focus areas related to insurance, testing, substance use, and HIV, with the goal of positive health outcomes for individuals and communities.
