Eligibility: Ages 18 or older. Must be a Medicaid recipient, must meet one of the following conditions: a significant mental illness two or more chronic health conditions such as asthma, diabetes, heart disease, mental health condition, or substance use disorder, must have risk factors that impact daily life functioning.

Application Process: Call or email to apply. Can go to www.hhuny.org and complete a referral online.

Required Documentation: Proof of Medicaid.

Fees: No fee for services.

Coverage Note: Monroe and Wayne Counties.

Villa of Hope offers Behavioral Health (mental health and substance use treatment), Care Coordination, Education, and Workforce Development programs to youth, adults and families.
