* Assists families of individuals with Alzheimer's disease and other forms of dementia with care management/caregiver consultation, as well as referrals and navigation to health care coordination, education & training, long-term care services, planning, respite services, support groups and health & wellness programming.

Eligibility: Caregivers of persons with Alzheimer’s, or related memory loss or dementia. Persons with mild Alzheimer’s or related memory loss or dementia (able to work with a navigator).

Fees: No fee for service. Donations accepted.

Lifespan of Greater Rochester provides older adults and caregivers with information, guidance, and support services. Lifespan also provides training and education for allied professionals, and the community.
