* Provides the following services for adults and children with multiple chronic health conditions, HIV/AIDS or a persistent mental health condition.
- Care Management.
- Transitional care.
- Patient and family support.
- Referral to community services.
Eligibility: Must have two or more chronic medical or mental health conditions, or one of the singly qualifying conditions. Must be enrolled in Medicaid. Children's Health Home: Birth to age eighteen. Adult Health Home: Age 18 or older.

Fees: Medicaid and Medicaid Managed Care. No fee to enroll.
