* Coordinates care and communication among providers and offers support during transitions of care. Services include:
  • Health promotion.
  • Patient and family support.
  • Referrals to community and social support services.

Eligibility: Age 18 and older. Must have chronic health conditions (physical or behavioral health). Must be eligible for Medicaid.

Fees: No fee for service.
