* Provides sensitive primary care to children living in foster care homes in Monroe County.

Eligibility: Monroe County DHS foster care children birth to 21 years of age.

Application Process: Referral from Monroe County Department of Human Services foster care system only.

Required Documentation: Call for information.

Fees: Accepts all insurance including Medicaid, Medicaid Managed Care.

Coverage Note: Monroe County.
