Eligibility: A member of a family may request a family care consultation. There is no maximum or minimum for how many family members are required for this service.

Required Documentation: No documents are required for this service.

Fees: No fee for service.

Coverage Note: Broome, Cayuga, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, St. Lawrence, Tioga, and Tompkins Counties.
