* Provides case management services to individuals, connects Medicaid HARP-enrolled individuals with employment, financial, vocational, educational, and more. Services are person-centered and are provided in a client’s home.

* Provides PEER Empowerment services.

Eligibility: Client must be enrolled in Medicaid and have an H1 or H9 code.

Application Process: Client or provider may call or email for information and referral form.

Required Documentation: Copy of current insurance card helpful.

Fees: No fee for services.
