* Provides one-on-one assistance with a professional case manager to help patients, caregiver, or providers resolve healthcare issues relating to access or barriers to care, insurance related issues, disability eligibility, and connection to practical and financial resources

 

Eligibility: -Be a U.S. Citizen or permanent resident of the U.S. -Have a confirmed diagnosis of a chronic or life-threatening disease OR need screening services related to symptoms of disease. -Be in active treatment, had treatment within the past 6 months, or going into treatment in the next 60 days (at a facility in the U.S. or U.S. Territories)

Application Process: Complete phone or online intake.

Fees: No fee for service.

The Patient Advocate Foundation (PAF) provides direct financial support for pharmaceutical co-payments to insured patients, including Medicare Part D beneficiaries, who financially and medically qualify.
