* Conducts community outreach to build trusting relationships with homeless population, and connects them to housing and other resources within the community.
Eligibility: Must be homeless.
Application Process: Call, walk-in, or visit website. Referrals, including self-referrals, are accepted.

Person Centered Housing Options provides case management, connection to housing programs offered through continuum of care, and resources to people experiencing homelessness or in danger of becoming homeless.
