* Works with Greater Rochester Health Home and Health Homes of Upstate New York to assist clients with getting connected to other resources to improve quality of life.
Eligibility: Must be homeless or at risk of homelessness.
Application Process: Call, walk-in, or visit website. Referrals, including self-referrals, are accepted.

Person Centered Housing Options provides case management, connection to housing programs offered through continuum of care, and resources to people experiencing homelessness or in danger of becoming homeless.
