  • Canes
  • Wheelchairs


* Accepts donations of new and gently used equipment. Does not offer loans or accept donations of: commodes, shower chairs, beds, lifts, power wheelchairs or any other type of medical equipment.

Eligibility: Resident of area served.

Application Process: Call ahead to verify availability of equipment. Does NOT offer loans or accept donations of: commodes, shower chairs, beds, lifts, power wheelchairs or any other type of medical equipment.

Required Documentation: Current Photo ID and proof of residency.

Fees: No fee for services.

Coverage Note: Henrietta, Chili, Scottsville, Wheatland, Hilton/Parma, Caledonia, Churchville, and Gates.

 

CHS Mobile Integrated Health Care provides Emergency Medical Services (EMS) to the residents of Henrietta, Chili, Wheatland, and Caledonia.
