* Provides individual and family support services for older adults that include:
  • Telephone information and assistance.
  • Geriatric wellness screening.
  • Case management.
  • Benefits advocacy.
  • Caregiver training.
  • Nutrition centers.
* Offers money management for older adults.

* Provides assessment and treatment for homebound older adults experiencing anxiety and/or depressive symptoms, in home counseling available, up to six sessions and maybe matched with volunteer coach to support independence and connecting with the community through the PATHS program.

Eligibility: Ages 60 years and older or their caregivers. PATHS/PEARLS: Experiencing anxiety and/or depression. Residing in a community setting (single home or apartment). Unable to access traditional counseling services.

Fees: Sliding scale for some services. PATHS: no fee for services.
