* Provides services to adults in recovery with persistent mental illness. Services include:

  • Rehabilitation services
  • Residential group homes as well as supervised apartments
  • Support services
  • Individual and group counseling
  • Recreational programs as well as educational and vocational programs

Eligibility: Resident of area served. Age 18 years and older.

Application Process: Referral must be made by a treatment provider.
