* Provides services to assist families/caregivers to enhance eligible child's development in cognition, communication, adaptive, physical or social/emotional domains.

Eligibility: Birth to Five years of age. Must have a confirmed developmental delay, have a medical or physical diagnosis with a high probability of developmental delay, have been screened and found to need further evaluation, suspected of or at high risk for developmental delays in one or more of the following domains: Cognitive, communication, motor, social/emotional and adaptive (self-help).

Application Process: Call to make a referral.

Required Documentation: Call for information.

Fees: No fee for service.
