* Offers benefits screening with assistance available online regarding eligibility requirements and application processes for Temporary Assistance, Food Stamps, Health Insurance, tax credits and other related programs by visiting www.mybenefits.ny.gov.

Eligibility: Income guidelines apply. Call or walk-in for information.

Application Process: Applications are available at both DHS locations (691 St. Paul Street and 111 Westfall Road), many community service agencies, on this website http://otda.ny.gov/programs/applications/ or can be mailed by calling the appointment line. Emergency assistance for food or rent must apply in person at 691 St. Paul Street.

Required Documentation: Call or walk-in for information.

Fees: No fee for service.
