Eligibility: Veteran of one of the US Armed Forces. Honorably discharged. Veterans of allied countries may be treated if care is authorized by said countries.

Application Process: Veterans may apply in person or by mail-in application. At the time of application, veteran should provide evidence of eligibility as an honorably discharged member of the armed forces by bringing a copy of DD-214 or military discharge papers.

Fees: Call for current information. Based on income and net worth levels.

Coverage Note: Western New York State including Monroe, Ontario, Wayne, Orleans, Genesee, Wyoming, Seneca and Livingston Counties.
