* Provides Veteran patients with general health care and social needs to allow them to continue living at home or with family. Services include:
  • Therapeutic exercise.
  • Recreational activities.
  • Health education & medication management.
  • Meals.
  • Care for patients with dementia.
  • Support and information for family caregivers.

Eligibility: Veteran of one of the US Armed Forces. Honorably discharged. Veterans of allied countries may be treated if care is authorized by said countries.

Application Process: Referral by physician. Veterans may apply in person or by mail-in application.

Required Documentation: Call for information. Evidence of eligibility as an honorably discharged member of the armed forces by bringing a copy of DD-214 or military discharge papers.

Coverage Note: Western New York State including Monroe, Ontario, Wayne, Orleans, Genesee, Wyoming, Seneca and Livingston counties.
