* Offers a range of wellness services such as traditional therapies like physical therapy and personal training, therapeutic fitness classes designed to be the next step after rehabilitation for a proactive way to maintain strength, flexibility and endurance as well as providing aquatic exercise programs.

Eligibility: Referral required for physical therapy.

Application Process: Call for information.

Required Documentation: None.

Fees: Fitness Memberships vary. Class and programming: fees vary. Some classes are free. Call for information.

Coverage Note: Primarily New York State. No one is turned away.
