* Provides self-directed services including fiscal intermediary and broker.


* Oversees residential houses providing support to persons for medical appointments, meal prep, socialization, budgeting and daily living skills.


* Provides Individual Support Services (ISS) housing/rent subsidiaries for persons with I/DD.

Eligibility: Diagnosis of intellectual/developmental disability (I/DD).

Application Process: Call office.

Required Documentation: Documented diagnosis of Intellectual Developmental Disability before age 22 years. Medicaid eligible.

Fees: No fee for service.

Coverage Note: Cayuga, Livingston, Monroe, Ontario, Orleans, Seneca, Yates and Wayne Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff

Languages: Spanish - Staff provides in-person interpreter, Materials/literature in language indicated, Website can be translated
