* Provides rehabilitation services after a surgery or a hospital stay, comprehensive rehabilitation services include:
- Physical Therapy.
- Occupational Therapy.
- Music Therapy.
- Speech and Hearing Services.
- Speech-Language Pathology.
- Independent Living Skills Training.
Language Access
Eligibility: Require short term rehabilitation. Viable discharge plan.
Application Process: Call for application and send PRI, Screen, medical history, summary and physical therapy notes.
Required Documentation: Call for information.
Fees: Medicaid. Medicare. Private pay. Long Term Care and HMO Insurance with approval.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.

St. John's Home provides 24-hour skilled nursing care and assistance with the activities of daily living. Available services are tailored to the resident's individual needs, and include nursing, medical, pharmacy, therapy, social work, pastoral care, therapeutic recreation, and beautician/barber services.
