* Offers day habilitation services. Includes: social and recreational activities, while receiving assistance with personal care and other areas of need, and individuals work toward goals and objectives that are derived from their valued outcomes.

* Provides at-home assistance to individuals to help them become increasingly independent and able to live in the least restrictive environment possible.

* Offers weekday and weekend respite. Weekend Respite provides an opportunity to socialize and enjoy a variety of recreational activities. Assistance with personal care, meal times, medication and other areas of need.

* Assists families with costs of goods or services directly related to the care of a family member with a developmental disability through grant funding.

Eligibility: Must be eligible for Medicaid and OMRDD eligible.

Application Process: Referrals may be made by individuals, parents, schools, health professionals, and/or other providers.

Required Documentation: Call for information.

Fees: Services provided through Medicaid.

Coverage Note: Cayuga, Cortland, Onondaga, Oswego, Seneca, Tompkins, and Wayne Counties.
