* Offers a Program of All-Inclusive Care for the Elderly (PACE), which works with seniors and their caregivers to arrange for and monitor all of their medical, social and daily living support services. Services include:
  • Medical care.
  • Home health aid services.
  • Transportation.
  • ElderONE PACE Day Centers.
  • Rehabilitation services.
  • Acute hospital care.
  • Nursing in-home care.
  • Durable medical equipment and supplies.
  • Nutritional counseling and meals.
  • Prescription drugs.
  • Recreation therapy.
  • Consumer Direct Personal Assistance Program (CDPAP).
