* Provides street outreach to violent and gang youth and their families.
  • Referrals to counseling, support and linkages to community resources and services to reduce youth-related violence
  • Monitors youths receiving services and supports positive progress
  • Violence prevention workshops and presentations

Eligibility: Ages 8-25 years. Involved in violent activity. Call for information.

Application Process: Referrals may be made by law enforcement, neighbors, friends and families, community agencies or self-referrals. All applications must be completed online.

Required Documentation: None.

Fees: No fee for service.

Coverage Note: City of Rochester.

Languages: Spanish.
