* Provides non-emergency patient transportation to health care services that include:
  • Diagnostic services. 
  • Outpatient surgery. 
  • Day treatment program. 
  • Physical therapy. 
  • Rehab. 

Eligibility: Call for information.

Application Process: Call at least 24 hours in advance of need. If cars and staff are available, may provide same day accommodations. For trips originating outside of Monroe county please call before sending paperwork.

Required Documentation: Call for information.

Fees: Third Party Insurance. Private pay. Medicaid.

Coverage Note: Primarily Monroe County. No one is turned away; however trip should start or end in Monroe County.
