Eligibility: Terminally ill whose prognosis is less than 3 months.
Application Process: May be referred by a hospice agency, hospital discharge planner, nursing home, private physician, doctor, or family and friends.
Required Documentation: Call for information.
Fees: No fee for service.
Coverage Note: Cayuga County, parts of Onondaga and surrounding counties.
Matthew House provides comfort care for terminally ill individuals who require 24 hour care and have a prognosis of three months or less.
