  • Caregiver/Family Supports and Services. 
  • Planned Respite and Crisis Respite. 
  • Community Self-Advocacy Training and Supports.

Eligibility: Ages birth-21 years. Must be a Medicaid, Medicaid Managed Care, or Medicaid Family of One recipient. Must meet Level of Care requirements.
