* Provides comprehensive developmental checks with referrals and follow-ups if needed. Screenings include:
  • Vision. 
  • Hearing. 
  • Speech. 
  • Dental. 
  • Social and emotional competence. 
  • Cognitive functioning. 
  • Physical development. 
  • Social determinants of health/education (including ACEs and Protective Factors). 

Eligibility: Ages 1 to 5 years (cut off is 60 months).

Application Process: Call or visit website for an appointment or more information.

Required Documentation: Consent form.

Fees: No fee for services.

Coverage Note: Chemung, Monroe, Steuben, Wayne, and Yates Counties.
