* Offers people with intellectual and developmental disabilities and their families assistance to navigate services and supports. Promotes informed choices and independent decision-making. Care Coordinators work continuously to connect people to the care, support, and opportunities.

  • Monitors circumstances and services during life transitions.
  • Coordinates access to medical, dental, and behavioral services.
  • Keeps track of safety and residential needs.
  • Referrals to community and social support services.
  • Creates a holistic Life Plan and adapts the plan as new goals and needs arise.

Eligibility: Diagnosis of intellectual and/or developmental disability prior to age of 22 and substantial handicap (deficits in the areas of social, communication, and daily living skills), autism spectrum disorders, cerebral palsy, epilepsy, neurological impairments, familial dysautonomia, and Prader-Willi syndrome are eligible conditions.

Application Process: Call for information.

Required Documentation: For OPWDD eligibility-Psychological evaluations (all years), social/developmental history or psychosocial report, IEP or 504 plan, physical, other medical records related to diagnoses when applicable. Call for more information.

Fees: Free through OPWDD eligibility and Medicaid navigation. Medicaid is required for enrollment.

Coverage Note: Allegany, Cattaraugus, Cayuga, Chautauqua, Chemung, Erie, Genesee, Livingston, Monroe, Niagara, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff
