Eligibility:
Must be referred by inpatient hospital care. Serious persistent mental illness. Ages 18 years of age or older. Individuals must be receiving mental health treatment in the community.

Thru: The Arc of Chemung-Schuyler

Eligibility:
6 years of age and older. OPWDD eligibility. Co-occurring mental health or behavioral health concerns that is causing stress within the person's system of care.
