Resident of area served. Newborn-18 years of age. Up to age 21 with a Developmental Disability.

Member agency must be a Foodlink member agency, a direct provider of food to the ill, children, and​/or needy, 501(c)3 organization, open to the public.

Uninsured or underinsured. 300% or less of the poverty level.

Visual impairment or legal blindness. Vocational​/employment services: For individuals who are legally blind, ages 14 and up, by referral of a New York State Commission for the Blind vocational rehabilitation counselor.

Individual impacted by a disaster event.

Six months of age or older.

The business must be located within New York State.
The business must have 1-50 full time equivalent (FTE) employees.
At least 30% of the employees must earn $45,450 or less in annual wages. The wage level is adjusted annually for inflation.
The business must not have provided group health insurance coverage to its employees within the last 12 months.
