-Be a U.S. Citizen or permanent resident of the U.S.
-Have a confirmed diagnosis of a chronic or life-threatening disease OR need screening services related to symptoms of disease.
-Be in active treatment, had treatment within the past 6 months, or going into treatment in the next 60 days (at a facility in the U.S. or U.S. Territories)

Ages 0-21. Has program that serves youth with Medicaid and a program that serves youth without Medicaid. Both must have a mental health diagnosis or history of trauma.

Resident of area served, child​/youth under the age of 21, enrolled in Medicaid. The child​/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Ages 6-21, receiving Medicaid, with a chronic medical diagnosis.

Must be Medicaid eligible. Call for information.
