The Children's Health Homes of Upstate New York, LLC (CHUNNY) operates a Health Home for Children using a model based upon the belief that every family has strengths and are the natural and most effective drivers of their care.

Provides case/care management to children and youth that are experiencing significant behavioral, medical, or social risk factors.
